Guarantee Letter (GL) Application Checklist

Please review the instructions and checklist before proceeding with your application.

Dear GL Applicant,

Please read the following instructions carefully. The process for the Office of the President Guarantee Letter (GL) application has been updated.

Effective March 2026, Sandbox Clinic will only provide the required:

  • Service Quotation
  • Certificate of Acceptance of the Guarantee Letter

Sandbox Clinic will no longer collect or submit application requirements to the Office of the President on behalf of applicants.

Important: All submission of requirements shall be completed directly by the applicant to the Office of the President.

The checklist below is provided solely as a guide to help ensure that your documents are complete prior to submission.

Role of Sandbox Clinic

Sandbox Clinic's role in relation to the Office of the President Guarantee Letter (GL) application is strictly limited to acting as the provider of services and the issuer of supporting documents, including but not limited to:

  • Service Quotation
  • Certificate of Acceptance of the Guarantee Letter

The review, evaluation, and approval of the GL application are undertaken exclusively by the Office of the President and/or the appropriate government authority.

Sandbox Clinic does not participate in, influence, or control the assessment or approval of the application beyond the issuance of the stated documents.

Application Checklist

Please prepare the following documents before proceeding with your GL application.

Documents to get from Sandbox Clinic
  • Sandbox Internship Program: GL Application Form ONLINE Application Form
  • Certificate of GL Acceptance from Sandbox Clinic
  • Quotation from Sandbox Clinic (Please note that this will take at least 5-7 business days as we need to review and confirm the doctor's prescription first before releasing the quotation.)
Documents from the Applicant
  • Letter of Request
  • Copy of Birth Certificate
  • Medical Certificate (issued within the last 2 months)
  • Doctor’s Prescription (listing all therapy/intervention needs, issued within the last 2 months)
  • Medical Evaluation coming from any of the following:
  • Developmental Pediatrician (evaluation may be 6 months to 1 year)
  • Neuro-Pediatrician
  • General Pediatrician with Developmental Screening
  • Rehabilitation Doctor
  • Orthopedic Doctor
  • Photocopy of 2 valid government-issued IDs with 3 specimen signatures (on same sheet as ID copies)
  • Driver’s License
  • PRC License
  • Passport
  • Voter’s ID
  • UMID
  • TIN ID
LGU Endorsement Documents
  • Certificate of Indigency (from Barangay)
  • Social Case Study Report (from MSWD/CSWD or DSWD)

Submission Instructions

Once completed, you may directly submit the documents in a brown envelope via LBC or deliver them directly to the Presidential Action Center.
